Add cover page info as an option for Request List

Other RV inspectors and I have customers who like the Request List feature, but the current output provides little to no information to identify the coach when it is submitted to a dealership. This can be resolved by adding an option to start the request list with the Inspection cover page.

In addition to the Cover Photo. Request to print out in the order of importance and not the order in which the item is located within the report. For example Life Safety Issues, Major, Minor and lastly Notice Comment. The Summary report prints in the correct order.
